Bridgwater Chiropractic requires all new patients complete their online intake forms at least 12 hours before their scheduled appointment time.
Appointments may be cancelled if forms are not completed. Patients who require assistance are encouraged to contact the office in advance so that suitable arrangements can be made.
First visits are typically not booked during evening (after 4pm) or weekend hours due to high demand. Exceptions are made on a case-by-case basis. If a first visit is scheduled at a premium time, we require a credit card on file to hold the booking.
A minimum of 24 hours for cancellation or rescheduling of any appointment is required. Repeated late cancellations, rescheduling or missed appointments will require a valid credit card to be held on your profile to book future visits, and may be subject to a fee of the full (adult) cost of the visit booked. Late arrivals may be required to either reschedule or shorten their visit times, and cancellation fees may be applicable.
We completely understand that unexpected things can come up - life happens! We just ask that you let us know as soon as possible so we can offer the time to someone else who may be waiting.
Your Extended Health Insurance policy is an agreement between you and your insurance company. In some cases, the clinic may be able to collect payment directly from your insurance company (i.e. Blue Cross, Sunlife, Manulife, etc). In the case of direct billing to your Extended Health provider, you will be required to present your EHB card and provide all pertinent information prior to billing. You are responsible for any balance incurred in the event your benefits coverage is rejected.
All Manitobans are eligible for partial coverage for chiropractic care (up to 7 visits per year) through Manitoba Health, when the clinic is provided a current copy of your health card. Visits will be charged at standard rates until valid Manitoba Health information is presented. You are responsible for any balance incurred in the event your Manitoba Health coverage is rejected.
In MPI or WCB cases, your claim number and date of injury is required to process the claim. Visits may be charged at standard rates until this information is presented, and in some cases, until confirmation of coverage is confirmed. We do require a credit card on file for the duration of your claim. It is recommended that you do not submit any charges to your EHB provider unless MPI or WCB coverage is denied. You are responsible for any balance incurred in the event your claim is rejected.
