Pain in the tailbone area can be surprisingly disruptive. Sitting, standing up, exercising, or even resting comfortably can become difficult when the coccyx is not moving or functioning as it should.

What Is the Coccyx (Tailbone)?

The coccyx is a small structure at the base of the spine. It helps support you when sitting and serves as an attachment point for pelvic floor muscles and ligaments. Even small changes in its movement can lead to discomfort.

Common Causes of Tailbone Pain

Tailbone pain may be related to:

  • Falls or direct impact 
  • Prolonged sitting 
  • Pregnancy and postpartum changes 
  • Repetitive strain (e.g., cycling) 
  • Pelvic floor tension 
  • Changes in low back or pelvic movement 

Often, more than one factor is involved.

How Tailbone Pain Is Assessed

Assessment begins with a detailed history. A focused exam may include:

  • Posture and movement assessment 
  • Evaluation of the low back, pelvis, and hips 
  • External palpation of the coccyx 
  • Functional testing (sitting and transitions) 

The goal is to identify the factors specific to you.

Treatment Options for Tailbone Pain

Most cases respond to conservative care, including:

  • External manual therapy 
  • Soft tissue techniques 
  • Exercise and mobility work 
  • Load and posture modifications 

Care is adapted based on your response and goals.

When Is Internal Coccyx Treatment Considered?

In some cases, symptoms persist despite appropriate care.

Internal coccyx treatment may be considered when:

  • Symptoms are ongoing and localized 
  • External treatment has not fully resolved the issue 
  • More direct assessment of coccyx movement is needed 


It is not routine and is always optional. If appropriate, the process is fully explained in advance.

  • A deep ache or pressure at the base of the spine 
  • Pain with prolonged driving or desk work 
  • Sensitivity to direct pressure over the tailbone 
  • Tension or discomfort through the pelvic floor or hips 
  • Occasional referral into the buttock or lower back 


Symptoms can vary depending on the underlying cause and how long the issue has been present.

How Chiropractic Care May Help

Your care always starts with a detailed history and physical exam to better understand your pain pattern, movement, and daily limitations. 

 

Care is directed at improving comfort, restoring movement, and addressing contributing mechanical factors.

Depending on your presentation, this may include:


  • Targeted manual therapy to the low back, pelvis, and surrounding joints 
  • Soft tissue techniques to reduce tension in the hips and pelvic region 
  • Movement and exercise strategies to support stability and control 
  • Guidance around sitting posture and activity modification 
  • Gradual return to activities that have been limited by pain 


When appropriate, care may also involve coordination with other providers, such as pelvic floor physiotherapy, to ensure a well-rounded approach.

 It may be helpful to have your tailbone pain assessed if:


  • Pain is interfering with sitting, work, or daily activities 
  • Symptoms have persisted beyond a few weeks 
  • Discomfort began after a fall, injury, or postpartum period 
  • Pain is not improving with basic rest or self-management 
  • You are unsure what is contributing to your symptoms
